UNIT I
MEDICAL RECORDS AND HEALTH INFORMATION TECHNICIANS I

   STANDARD COMPETENCE
Understand about MEDICAL RECORDS AND HEALTH INFORMATION TECHNICIANS I and the function of Yes/No and WH questions in sentences properly.
   BASIC COMPETENCE
  1. Understand the career of Medical Records and health information technicians I.
  2. Understand how to use Yes/no and WH questions properly in sentences, paragraphs, composition in writing (passive English) and oral (Active English) properly.
  3. Understand more about how to introduce and its supported vocabularies in telling story to support speaking skill

   INDICATOR
After teaching and learning process students are expected to be :
  1. Able to explain about MEDICAL RECORDS AND HEALTH INFORMATION TECHNICIANS I and the function of Yes/No and WH questions in sentences.
  2. Able to use Yes/No and WH questions properly in sentences, paragraph, composition (passive English) and oral (active English).
  3. Able to introduce well.

A.    READING
MEDICAL RECORDS AND HEALTH INFORMATION
TECHNICIANS I
Standard Occupational Description: Compile, process, and maintain medical records of hospital and clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. Process, maintain, compile, and report patient information for health requirements and standards.
Medical Records and Health Information Technician positions in the Commonwealth are assigned to the following Roles in the Administrative and Office Support Career Group:
Administrative and Office Specialist III, but office specialist III will not be discussed in this book.
Medical Records and Health Information Technicians within the Commonwealth are all located within the Administrative and Office Support Career Group, individuals may want to pursue other opportunities within the Commonwealth depending upon individual training, education, knowledge, skills, abilities,. In this unit we will just discuss and study about skills and knowledge., the others will be discussed in the next unit.

1.      SKILLS
Note: The technical and functional skills listed below are based on general occupational qualifications for Medical Records and Health Information Technicians commonly recognized by most employers.  Typically, you will not be required to have all of the skills listed to be a successful performer.  Recruitment and selection standards for an individual state job must be based on the specific knowledge, skills, and abilities for that job as indicated in the job announcement and job description in the Employee Work Profile.
a.       Active Listening -- Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
b.      Reading Comprehension -- Understanding written sentences and paragraphs in work related documents.
c.       Time Management -- Managing one’s own time and the time of others.
d.      Speaking -  Talking to others to convey information effectively.
e.       Writing -- Communicating effectively in writing as appropriate for the needs of the audience.
f.       Active Learning -- Understanding the implications of new information for both current and future problem solving and decision-making.
g.      Instructing -- Teaching others how to do something.
h.      Critical thinking--Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
i.        Social perceptiveness -- Being aware of others’ reactions and understanding why they react as they do.
j.        Judgment and decision making -- Selecting and using training/instructional methods and procedures appropriate for the situation when learning or teaching new things.
2.      KNOWLEDGE

The Knowledge of:
a.       Clerical- knowledge of administrative and clerical procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and other office procedures and terminology.
b.      Customer and Personal Service – knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
c.       English Language-knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
d.      Computers and Electronics -- Knowledge of Computer hardware and software, including applications and programming.
e.       Coding and indexing of diseases and medical procedures.

B.     GRAMMAR
In this section you are going to study about yes/no questions and WH questions:
1.      YES-NO QUESTIONS
These are called yes-no questions because the answer (which may be expressed or just implied) is yes or no, to make a statement into a yes-no questions, it is simple but put the operator (am, is, are, was, were, do, does, did, have, has, will, etc) before the subject. Yes-no questions usually have rising intonation.

2.      WH – QUESTIONS
These questions for which the answer is more than yes or no; there must be some information in the answer. Wh-questions begin with an integrative like who, what, when, why, where, etc. obviously, there are three different rules in the part.

a.      Who or what is subject questions
A subject question is one in which the subject is unknown

Note: it is not correct to say:

b.      Whom and what in complement questions

Note: Although in speech, most people use who rather than whom in these questions, in correct written English, you should use whom to indicate that the question word comes from the complement position.

c.       When, Where, How and Why questions
These questions are formed the same as complement questions.

When we are discussing about Wh-question, in some terms, we also find another question used Wh-questions. Usually is called embedded question is one which is included in a sentence or another question. The word order is not that off typical questions except for subject question. Study the following rule.
Subject + Verb (phrase) + Wh-questions + Subject + Verb
Note: there must be an auxiliary between the question word and the subject in an embedded question.
Question:
o   Where will the meeting take place?
Embedded question:
We haven’t ascertained where the meeting will take place
o   Where              : Wh-question
o   The meeting    : Subject
o   Take place       : Verb phrase
Question:
Why did the plane land at the wrong airport?
Embedded question:
The authorized cannot figure out why the plane landed at the wrong airport.
Auxiliary + Subject + Verb + Wh-questions + Subject + Verb
Examples:
1.   Do you know where he went?
2.   Could you tell me what time is it?
3.   Can you give a reason why Bambang bought a car?

3.      Certain Note
a.   Wh-question can be single or phrases. Phrases include:
Whose + noun, how many, how much, how long, how often, what time, and what kind.
·         I have no idea how long the interview will take.
·         Do they know how often the bus runs at night?
·         Can you tell me how far the museum is from Solo?
·         The teacher asked us whose book was on his desk.

b.   There is no change in the order of subject position questions because the wh-question is functioning as the subject.
·         Who will paint that picture?
·         They can’t decide who will paint that picture.
·         Whose car is parked in the lot?
·         The police can’t determine whose car is parked in the lot
c.    This is how to form wh-questions:
1.      Put the sentence element which contains the wh-word at be beginning of the sentence.
2.      If the element containing the wh-word is object, complement, or adverbial, place the operator is in front of the subject.
For example:
a)      Wh-element is odbject.
Rina asked a question. What question did Rina ask?
b)     Wh-element is complement
These animals are very clever. How clever are these animals?
c)      Wh-element is adverbial
They’ll leave tomorrow. When will they leave?
3.      If the element containing the wh-word is the subject, the verb phrase remains the same as in the corresponding statement, and no do-contraction is necessary.
