A.    READING
MEDICAL RECORDS AND HEALTH INFORMATION
TECHNICIANS II
After discussing skills and knowledge in the previous unit, in this unit will continue discussing about the two more individual training in medical records namely abilities, tasks and jobs activities of medical records
ABILITIES, TASKS and JOBS Activities (Technical and Functional Expertise)
1.      ABILITIES
Note: The technical and functional abilities listed below are based on general occupational qualifications for Medical Records and Health Information Technicians commonly recognized by most employers.  Typically, you will not be required to have all of the abilities listed to be a successful performer.  Recruitment and selection standards for an individual state job must be based on the specific knowledge, skills, and abilities for that job as indicated in the job announcement and job description in the Employee Work Profile.
The Ability to:
a.     Oral comprehension – the ability to listen and to understand information and ideas presented through spoken words and sentences.
b.     Written comprehension – the ability to read and understand information and ideas presented in writing.
c.     Oral expression -- the ability to communicate information and ideas in speaking so others will understand.
d.    Information ordering – the ability to arrange things or actions in a certain order or pattern according to a specific rule or set of rules (e.g., patterns of numbers, letters, words, pictures, mathematical operations).
e.     Near vision – the ability to See details at close range (within a few feet of the observer).
f.      Speech clarity – The ability to speak clearly so others can understand you.
g.     Category flexibility -- the ability to generate or use different sets of rules for combining or grouping things in different ways.
h.     Speech recognition – the ability to Identify and understand the speech of another person.
i.       Written expression – the ability to communicate information and ideas in writing so others will understand.
j.       Selective Attention -- The ability to concentrate on a task over a period of time without being distracted.
k.     The ability to tell when something is wrong or is likely to go wrong. It does not involve solving the problem, only recognizing there is a problem.
2.      TASKS
Note:  The following is a list of sample tasks typically performed by Medical Records and Health Information Technicians.  Employees in this occupation will not necessarily perform all of the tasks listed.
Tasks
Protect the security of medical records to ensure that confidentiality is maintained. Process patient admission and discharge documents. Review records for completeness, accuracy and compliance with regulations. Compile and maintain patients' medical records to document condition and treatment and to provide data for research or cost control and care improvement efforts. Enter data, such as demographic characteristics, history and extent of disease, diagnostic procedures and treatment into computer. Release information to persons and agencies according to regulations.
Plan, develop, maintain and operate a variety of health record indexes and storage and retrieval systems to collect, classify, store and analyze information. Manage the department and supervise clerical workers, directing and controlling activities of personnel in the medical records department. Transcribe medical reports. Identify, compile, abstract and code patient data, using standard classification systems.
Resolve/clarify codes and diagnoses with conflicting, missing, or unclear information by consulting with doctors or others to get additional information and by participating in the coding team's regular meetings. Train medical records staff. Assign the patient to one of several hundred "diagnosis-related groups", or DRGs, using appropriate computer software. Post medical insurance billings. Process and prepare business and government forms. Contact discharged patients, their families, and physicians to maintain registry with follow-up information, such as quality of life and length of survival of cancer patients.
Prepare statistical reports, narrative reports and graphic presentations of information such as tumor registry data for use by hospital staff, researchers, and other users. Consult classification manuals to locate information about disease processes. Compile medical care and census data for statistical reports on diseases treated, surgery performed, and use of hospital beds. Develop in-service educational materials.
3.      Job Activities of Medical Records
a.       Getting information -- observing, receiving and otherwise obtaining information from all relevant sources.
b.      Interacting with computers- using computers and computer system (including software and hardware) to program, write software, set up functions, enter data or process information.
c.       Communicating with supervisors, peers, or subordinates- providing information to supervisors, co-workers and subordinates by telephone, in written form, e-mail, or in person.
d.      Updating and using relevant knowledge – keeping up-to-date technically and applying new knowledge to your job.
e.       Evaluating information to determine compliance with standards.-Using relevant information and individual judgment to determine whether events or processes comply with laws, regulations or standards.
f.       Documenting/ recording information – entering, transcribing, recording, storing, or maintaining information in written or electronic/magnetic form.
g.      Establishing and maintaining interpersonal relationships- developing constructive and cooperative working relationships with others, and maintaining them overtime.
h.      Handling and moving objects- using hands and arms in handling, installing, positioning and moving materials and manipulating things.
i.        Organizing, planning, and prioritizing work- developing specific goals and plans to prioritize, organize, and accomplish your work.

Exercises
I.       Answer these questions below!
1.      In your opinion, what is the most important task of medical record? Why? Please explain it clearly!
2.      In the ability parts, medical record coder must be communicative, can you explain why he/she must be communicative?
3.      Do you think English is Important for medical recorder? Why do you think like that?
4.      From the explanation of the abilities above, what kind of characteristic must be had by medical record coder?
5.      Which one is more important, ability or diploma? Why?
6.      Explain about the job activities of medical records! Do you think, those are easy jobs or not? Why?

B.     GRAMMAR

PRONOUNS
There five forms of pronouns in English namely Subject pronouns, complement pronouns (object pronouns), Possessive pronouns, Possessive adjective and Reflexive pronouns.
1.      Subject Pronouns
Subject Pronouns are used as a subject and in the subject position of a sentence or after than, as, that, and after the Verb are.

Complement pronouns occur in complement position or in object position

3.      Possessive Adjective
Possessive adjective is not the same as possessive pronoun. Possessive forms indicate ownership. These are the possessive pronouns.

4.      Possessive Pronouns
The pronoun cannot precede a noun because these pronouns replace the noun.

 5.      Reflexive Pronoun
These pronouns usually follow the verb and indicate that the subject is both giving and receiving the action.

HOME REMEDIES
When people have a cold, a fever or flu, they usually go to the doctor for help or they get some medicine from the drugstore. But many people also use some remedies for common illnesses.
Lots of people drink hot chicken soup when they have a cold. They find it clears the head and the nose. Some people rub oil on the chest for a cold. Other people drink a mixture of red pepper, hot water, sugar, lemon juice, and milk or vinegar. Here are some simple home remedies.
Bee stings and insect bites: wash the sting or bite. But put some meat tenderizer on a handkerchief and then put it on the bite for half an hour.
Burns         : put the burn under cold water or put a cold handkerchief on it. But don’t put ice on the burn.
Coughs      : drink warm liquids or take some honey.
Indigestion: drink some water with a teaspoon of baking soda in it.
Insomnia   : drink a large glass of warm milk.
Development:
